Companies from outside the emirate: to provide company’s accreditation certificate from the relevant municipality and type of accreditation.
Specialized certificates for service providers according to the type of service accreditation to ensure their academic qualification and adequate experience in providing such services in the emirate.
Identification papers that prove the service provider's work with the company.
company's work history file in the state or emirate.
Facility APHP accreditation certificate
In case of accreditation service of private laboratories: an official undertaking to be submitted by the laboratory to send all non-conformant laboratory analyzes to the Department.
In the event of provision of accreditation service of drinking water tanks disinfection and cleaning companies: to provide list of chemicals and tools used for cleaning and disinfection.
In the event of provision of plan execution companies accreditation, providing samples of plans previously executed by the company to be provided for a number of activities specified in the service request.
